Separate the form from the return policy
The form is a structured way to identify a return. It should not override the merchant's website policy, promise a refund or imply that every submitted item is eligible.
Use careful language such as request, review and authorization. Link or direct the customer to the current policy rather than copying a long policy that may become outdated.
Capture only fields that support the workflow
Order reference, customer name, item, variant, quantity and return reason usually support basic identification. Add fields only when someone on the receiving side will use them consistently.
Avoid requesting unnecessary personal or payment information. The document may travel inside a parcel and should contain only what the merchant needs for the stated process.
Make the receiving handoff easy to scan
Use checkboxes for items and common reasons, with space for a short note when needed. The receiving team should be able to compare the parcel, form and order without decoding a dense paragraph.
Keep internal disposition fields on an internal document if they are not intended for the customer. Customer and warehouse workflows can share visual rules without sharing every field.
Review policy-dependent wording regularly
Return windows, authorization steps, shipping instructions and exclusions can change. Assign ownership for reviewing the template whenever the merchant's policy changes.
Test the form with partial returns and multiple quantities. Confirm that the printed version provides enough room for handwritten selections.

Does a return form authorize a refund?
No. It documents the customer's request and the returned items. Eligibility, inspection and refund decisions remain subject to the merchant's policy and process.
Should the return policy be printed in full?
Usually a concise instruction and reference to the current policy are safer to maintain. If policy text is printed, the merchant must keep the template synchronized with any policy change.
